The Central Community Health Partnership (CCHP) is dedicated to helping MassHealth enrollees with Behavioral Health challenges or complex Long-term Service and Support (LTSS) needs overcome barriers that interfere with quality of life and good health.
Working closely with ACO’s (Accountable Care Organizations) and MCO's (Managed Care Organizations), healthcare providers and social service agencies, we coordinate services for eligible MassHealth members to ensure seamless access to the care you need.
Who We Are
Central Community Health Partnership is a program of Open Sky Community Services in partnership with three agencies -- Center for Living and Working, Elder Services of Worcester Area Inc., and Venture Community Services.
Our staff team includes licensed clinical care managers, registered nurses and care coordinators -- all working together to help Primary Care Physicians and their patients like you. Our partners have decades of experience serving people who have mental health challenges, substance use disorders, intellectual/developmental disabilities, housing insecurity or homelessness, medical conditions and/or other challenges to help them access services that meet their needs.
Helping ACO's and MCO's Provide High Quality Services for You
The Central CHP’s collaborative group of partners coordinate member care so that the right services and supports are available at the right time to improve quality of care and increase member satisfaction. We provide both Behavioral Health and Long-Term Services and Supports (LTSS) Community Partner services.

Elder Services of Worcester Area, Inc. (ESWA) is a private, non-profit corporation that was created by the Commonwealth of Massachusetts in 1974. The agency is one of a network of 24 “Aging Service Access Points (ASAP)” across the state and serves as an entry point for services and benefits. Learn more
